Here's an example of the filled-out form, using my character Intombazane as reference.
Your alias/internet handle: Aquillaflyer.
Your age: 19.
Your gender/pronouns: Female.
Activity level: Probably about 15-18, I plan to be as active as I can.
Character name: Intombazane.
Species: Homotherium (H. latidens in specific).
Size: 3'5" at the shoulder, about 6 feet long, roughly 380 pounds.
Character age: Young adult, in the ballpark of 3-5 years.
Character gender: Female.
Group affiliation: None.
Character sexuality: Even I don't know for sure.
Character's appearance:
Intombazane is pretty typical for a homotherium when it comes to size and build. Athletic-looking with partially-defined muscles and large, pointed ears. Her fur is mostly tan, similar in color to that of a cougar, with a whiteish underbelly and faintly darker hook-like markings. She has a long mane-like ruff running from her forehead to the middle of her back. It's far darker than the rest of her fur, with it being the darkest at the tips. Her ears are the same color as her mane, with a gradual light-to-dark fade and a white spot on the back of each ear like in many species of cat. Her eyes are pale blue.
Character's personality:
Intombazane is not one for social interaction, suffering from a lot of anxiety and awkwardness whenever she tries to interact with others. Although not universally timid, she often appears so whenever she is forced to interact with anyone. Preferring to run when confronted with danger, she will fight to the death if she has little other choice, and does not fear inflicting grizzly injuries on any unfortunate opponent. Known to be a pessimist, she tries her best to laugh at many of the rough patches in her life. Those events that cannot be paved over with bad jokes are rarely brought up.
Brief roleplay sample:
She'd been running for a long time, worried that whatever it was that had been chasing her. But now she was tired, worn, looking for a place to rest, even if it was just for a minute. Her paws ached, and her lungs felt as if they couldn't pull in enough air. She couldn't tell how long she'd been on the run for, the exhaustion had stolen much of her ability to figure that out, not like she cared much. All she wanted was to find somewhere that she was safe, where she could sit down for a moment and just breathe. But here didn't seem to be the place. There were no trees, there was no grass, no natural formations. Not even rocks larger than her paw. Just endless, cracked earth home to nothing but windblown dust that stung the eyes and made breathing even harder. A thousand stars, untouched by any light pollution, impassively watched her as she made her way across the desolate landscape. Still, she ran. And through the dust and the darkness of the night, she saw something in the distance. It wasn't trees, but a forest of stones, many shaped into spikes by that same wind and dust that now faintly obscured their shapes. Knowing that the sunrise was not far away, and of the distance of anywhere else to hide, she summoned all her remaining energy continued running with the hope she'd make it there soon.
This is how I write most of my posts, but I do try and put my character's name in there at least a couple times. Many of my posts are also far shorter. This is more of what a starting or entry post would look like, as I'm trying to set the scene for the people I'm playing with.
